Community Partner Agreement Form

As a Community Partner, I agree to:

  1. Have at least one (1) trained/qualified bicycle helmet fitter present when distributing helmets, to educate helmet recipients and demonstrate the proper fit and function of the bicycle helmet.
  2. Accept delivery of safety materials at no charge to the Community Partners. For helmets, the delivery address must be a commercial address staffed during regular business hours. 
  3. Distribute safety materials free of charge to individuals without regard to race, sex, disability, age, veteran status, national origin, religion, marital status, or political affiliation.
  4. Submit appropriate reporting after each event where safety materials are distributed. Failure to do so may disqualify you from receiving resources in the future.
